Job summary

Senior Services Associates in Crystal Lake, IL, seeks a Full-time APS investigator to conduct abuse, neglect and financial exploitation investigations among older adults and adults with disabilities. You will perform face-to-face assessments, determine substantiation, link clients to services, and coordinate with law enforcement as needed.

Requires a Bachelor’s or similar credential in health/social fields, with experience; travel within the county and occasional overnights may be required.

Qualifications

• Bachelor’s Degree in health, social services, social work, gerontology, criminal justice, public administration or related field.
• RN/BSN or BA/BS in health, social sciences, social work or related field with one year experience; or LPN with two years in health or human services.
• IDOA certification training and online forms training required within six months of certification; Phase II certification training within six months.

Responsibilities

• Receive APS report intakes per IDOA guidelines.
• Conduct investigations of APS cases per IDOA guidelines.
• Assess reports of abuse, neglect, exploitation or self-neglect and determine substantiation.
• Maintain detailed case notes and complete forms promptly.
• Collaborate with CCP case managers on mutual client cases.
