Job summary

Mercyhealth in Rockford, IL seeks an experienced APS Caseworker to investigate abuse and neglect among older adults, assess needs, plan services, and coordinate resources. You will interview victims and collateral contacts, document thoroughly, and follow up monthly to ensure ongoing safety and well-being.

Responsibilities include court procedures, coordination with waiver services, and participation in multidisciplinary teams.

Qualifications

• Bachelor’s or Graduate Degree in health or human services, social work, health care administration, gerontology, criminal justice, or nursing.
• Associate degree with four years' experience in health or human services or related field.
• Active LPN license or willingness to obtain within Mercyhealth timeframe.

Responsibilities

• Investigates abuse, neglect, abandonment, financial exploitation or self-neglect within required IDoA timelines.
• Interviews victims, involved family, providers and collateral contacts.
• Provides case work services: assess needs, develop plans, link to resources and monitor outcomes.
• Conducts monthly follow-ups to evaluate service sufficiency and risk.
• Completes IDoA/APS paperwork accurately and timely.
• Negotiates challenging family situations with appropriate supervision when needed.
• Utilizes court system for Orders of Protection, evictions, guardianships, etc., and may testify in court.
• Coordinates with Waiver services per IDoA policies and participates in team meetings.
