Job summary

Qualifications

• Experience conducting home visits and needs assessments.
• Ability to manage cases and coordinate with service providers.
• Knowledge of adult protective services processes and reporting requirements.

Responsibilities

• Conduct home visits to assess clients' needs and access services.
• Complete an investigation assessment for adult abuse and interviewing involved parties.
• Determine need for emergency intervention services and provide crisis intervention.
• Coordinate assistance from other providers on behalf of clients.
• Meet required timeframes for investigations, casework, and follow-up.
• Consult with supervisor on case substantiation decisions.
• Develop a case plan with client and supervisor.
• Provide services including OPs, guardianships, and emergency shelter as needed.
• Process referrals to other agencies and private individuals.
• Complete documents accurately and submit to IDOA as required.
• Monitor and reassess ongoing risk for each client.
• Prepare and present cases for M-TEAM meetings and implement recommendations.
• Collaborate with staff and other agencies to reach hard-to-reach clients.
• Maintain timely documentation of services provided.
• Participate in agency events and outreach to promote services.
