Job summary

The Lancaster County Office of Aging is seeking a Caseworker to provide care management for OPTIONS consumers and those on the waiting list. This role develops comprehensive Individualized Service Plans and conducts home visits and calls to ensure services meet consumer needs.

The position requires a Bachelor’s degree in social sciences (or equivalent) and relevant experience; DMV checks and background screenings are required, with ongoing training and compliance.

Qualifications

• Six months of experience as a Caseworker Trainee or equivalent program.
• Bachelor’s degree with 12 credits in sociology, social welfare, psychology, gerontology or related social sciences.
• Any equivalent combination of experiences and training including 12 credits in related social sciences.

Responsibilities

• Develop and document comprehensive ISP based on assessed needs.
• Perform extensive data entry and case documentation per agency standards.
• Provide Back Up Protective Service Investigative services to reduce risk.
• Travel to consumer homes/meetings using own transportation as required.
