Job summary

Community Mental Health Affiliates, Inc. seeks a Peer Navigator for the IMPACT program in New Britain, CT.

The role focuses on outreach, recovery support, and empowerment for clients with behavioral health and substance use needs, using personal recovery experience to inspire change and connecting clients to wrap-around services. The position requires CCAR Recovery Coach certification, a high school diploma or GED, and 1–2 years of related experience.

Qualifications

• Personal recovery experience used to inspire clients.
• Experience facilitating client support groups.
• Ability to coordinate with care teams and engage with wrap-around services.
• Knowledge of local community resources and emergency/crisis response.
• Maintain timely documentation in line with CMHA policies.

Responsibilities

• Shares personal recovery experience to provide hope and empowerment to encourage and facilitate change within clients using individual and group modalities including 12 step groups.
• Facilitates client support groups.
• Creates recovery oriented rapport with individuals with mental health or co-occurring disorders and connects them to wrap around services.
• Actively participates in community meetings to advocate for client and to ensure the provision of integrated care.
• Coaches and provides supervision/direction with daily challenges such as personal hygiene, nutrition, housekeeping, personal budget and household management.
• Collaborates with care team on client engagement and treatment recommendations.
• Provides encouragement and recovery knowledge to family members/natural supports.
• Carries a caseload based on program need.
• Ensures timely documentation is maintained in accordance with CMHA policies and procedures.
