The role focuses on outreach, support, and empowerment of clients with behavioral health or substance use challenges, helping them access wraparound services and recovery-oriented resources. You will work Monday through Friday, 9:00 AM to 5:00 PM, at 233 Main St., New Britain, CT 06051, contributing to recovery goals and community

Responsabilités

• Shares personal recovery experience to provide hope and empowerment to encourage clients.
• Facilitates client support groups.
• Create recovery oriented rapport and connect clients to wrap around services.
• Participates in community meetings to advocate for client and integrated care.
• Coaches and provides supervision with daily challenges such as personal hygiene and budgeting.
• Collaborates with care team on client engagement and treatment recommendations.
• Encourages and educates family members/natural supports.
• Carries a caseload based on program need.
• Maintains timely documentation in CMHA policies.
• Performs other related duties as assigned.
