Job summary

Community Mental Health Affiliates, Inc. seeks a Peer Navigator for the IMPACT program in New Britain, CT.

You will provide outreach and support to clients with behavioral health and substance use disorders, drawing on lived recovery experience to empower individuals toward recovery goals. The role involves coordinating with emergency and treatment facilities, facilitating groups, and helping clients access wrap-around services.

Responsibilities

• Provide outreach and ongoing support to potential and current clients.

• Facilitate client support groups and recovery-oriented activities.
• Build recovery-focused rapport and connect clients to wrap-around services.
• Collaborate with care team on engagement and treatment recommendations.
• Maintain timely documentation per CMHA policies.
