Job summary

Qualifications

• Bachelor's Degree in Healthcare Administration, Public Health or related field.
• Minimum of five years of experience in healthcare.
• Minimum of five years experience quality improvement and quality assurance.
• Knowledge of FQHC operations, HRSA requirements, NCQA standards, and UDS reporting measures.
• Working knowledge of Electronic Health Record systems.

Responsibilities

• Coordinate services, assessment, monitoring and evaluation of health care needs of high-risk patients.
• Oversee and report on effectiveness of population health programs, case management and chronic disease initiatives.
• Lead efforts to maintain national accreditations (e.g., PCMH).
• Coordinate quarterly departmental quality chart reviews and record results.
• Participate in health plan quality/population health meetings.
• Coordinate quarterly on-site quality rounds and staff education forums.
• Collaborate to develop and maintain quality improvement initiatives and policies.
