Job summary

Sertoma Star Services seeks a Community Services Coordinator to develop and coordinate Home-Based and Day Services for individuals with intellectual and developmental disabilities and mental health needs.

The role is hybrid remote/office with flexible days, evenings, and weekends as required. Requires a bachelor’s in human services and QIDP certification or plan to obtain within six months, plus relevant experience and strong communication skills.

Qualifications

• Bachelor’s in human services required; alternative degree with 30 credit hours in human services approved for QIDP.
• DHSA-certified QIDP or ability to complete certification within 6 months.
• Minimum of one year experience with individuals with developmental disabilities and/or mental illness preferred or achievable within 6 months.
• Strong verbal and written communication skills.
• Valid driver’s license and current auto insurance with acceptable driving record.
• Proficient in computers and Microsoft Office Suite.

Responsibilities

• Develop and implement support services for Sertoma Consumers in Home-Based and Day Services.
• Coordinate Self-Direction, Community Day Services, or related services based on consumer needs.
• Provide direction and training to staff, families, guardians, and volunteers for service implementation and data collection.
• Manage and audit consumer file documentation, agency reports, and billing records for compliance.
• Ensure services comply with DHS Rules, CARF standards, and agency policies.
• Oversee SDA service delivery, monitor agreements and authorizations via visits and calls.
• Assist in arranging and managing PSW services; train on recruitment and communication skills.
• Complete and audit Sertoma documentation, Medicaid waivers, and budget tracking for clients.
