Job summary

Person Centered Services Care Coordination Organization, LLC is seeking a Health home–oriented Care Coordinator to coordinate and advocate for individuals with intellectual and developmental disabilities. The role focuses on health and behavioral healthcare, community supports, and other services, using a person-centered, holistic approach to ensure needs are met.

Applicants with direct care, coordination, or advocacy experience are encouraged to apply.

Qualifications

• Bachelor's degree with 2 years relevant experience OR a Licensed Registered Nurse with 2 years relevant experience OR Master's degree with 1 year of relevant experience
• Minimum of a Bachelor's degree is required by OPWDD in NYS
• Relevant experience consists of providing direct care or support, coordination, and/or advocacy for anyone (examples: Direct Support Professional, Teacher, Paraprofessional, Counselor, Social Worker, Care Manager)

Responsibilities

• Develops a comprehensive, person-centered Life Plan with the individual and their circle of support, as well as their entire service provider team
• Effectively manage a tiered caseload, while tailoring services to individual needs
• Completes enrollment and eligibility documentation
• Meets with individuals in their homes, physician/provider offices, and other public places to conduct assessments and provide services
• Commits to a respectful, just, and supportive environment for individuals and coworkers aligning with DEI commitments
