Job summary

The Salvation Army seeks a POH Coordinator to supervise and deliver strength‑based case management for families and individuals, coordinating outreach, assessments, and referrals in local communities.

Responsibilities include intake, case planning, home visits, documentation, and collaboration with staff to promote housing stability, income growth, and self-determination. This full-time role requires strong communication and sensitivity to diverse communities.

Qualifications

• Degree in human service area; bachelor’s preferred; extensive strength-based case management experience may substitute for a degree.
• Minimum of two years of case management experience in a comparable social service program with multi-faceted interventions.
• Experience and/or strong interest in community outreach, organization and community capacity development.
• Must work with diverse populations respectfully and effectively.
• Excellent oral and written communication skills; ability to maintain confidential information.

Responsibilities

• Coordinate Pathway of Hope (POH) services using strength-based case management.
• Perform intake, develop case plans, and monitor progress for housing stability and economic resources.
• Maintain case files with assessments, plans, progress notes, and discharge summaries.
• Provide referrals, advocacy, and connections to community resources; conduct home visits.
• Ensure data entry in SIMS and prepare monthly POH activity reports.
