Job summary

The Salvation Army Independence Corps is seeking a dedicated Pathway of Hope (POH) Coordinator to support families with minor children. You will perform outreach, screen applicants, conduct intake, and develop case plans designed to improve housing stability, income, and self-determination.

Responsibilities include coordinating services, maintaining documentation in SIMS, and linking families to community resources.

Qualifications

• Degree in human services or related field; BA preferred.
• Minimum 2 years of case management experience in social services.

Responsibilities

• Coordinate outreach to identify eligible families for POH.
• Perform intake, assessments, and develop individual case plans.
• Provide referrals and coordinate services with community partners.
• Conduct home visits and monitor progress toward goals.
• Maintain case files and documentation in SIMS.
• Prepare discharge plans addressing housing and financial stability.
