Job summary

PATH (People Assisting The Homeless) is seeking a Case Manager in Los Angeles, CA to support clients through homelessness services, interim housing and transition to permanent housing. You will assess needs, coordinate services, and help clients access healthcare, social support and housing resources.

Role emphasizes trauma-informed care, motivational interviewing, and collaboration with the Interim Housing team to break the cycle of homelessness and promote housing stability.

Qualifications

• Analytical skills to gather and interpret data and identify trends.

Responsibilities

• Conduct intake and individualized needs assessment for all participants and develop ISP addressing housing goals and income.
• Provides referrals and support to help participants achieve ISP goals.
• Completes program-specific assessments (e.g. VI-SPDAT).
• Evaluates strengths and challenges to address short and long-term goals; conducts a 90-day review.
• Maintains participant files with HIPAA-compliant confidentiality.
• Develops supportive client relationships to advance housing goals.
• Provides crisis intervention and risk assessment services to increase safety.
• Uses motivational interviewing and trauma-informed care in service delivery.
• Maintains knowledge of community resources to provide wrap-around services.
