Job summary

Community Action Partnerships of Western Nebraska (CAPWN) seeks an Immunization Coordinator & Primary Care RN to manage immunization and provide clinic nursing services within a Patient-Centered Medical Home framework.

The role leads the Immunization Program, ensures vaccine policy compliance, educates patients, coordinates care, and collaborates with providers to deliver accessible, high-quality care with HIPAA compliance.

Qualifications

• Current BLS Certification.
• Valid Nebraska driver's license.
• RN experience in clinic/public health preferred.
• Ability to manage immunization programs and vaccine compliance.

Responsibilities

• Oversee, coordinate, and implement all program activities for the Immunization Department.
• Maintain electronic health records and ensure program compliance.
• Coordinate patient appointments and follow-up care.
• Care-manage chronic diseases and assist with referrals.
• Collaborate with providers and staff to promote patient outcomes.
• Maintain vaccine inventory and cold-chain documentation.
