CAPWN is seeking an Immunization Coordinator & Primary Care RN in Gering, NE. The role combines immunization program management with frontline nursing in a busy clinic setting.

Community Action Partnerships of Western Nebraska (CAPWN) is a non-profit community-based health and human services organization that serves low-income, disadvantaged and those unable to meet their needs through other sources. The agency was established in 1965 and provides services to over 9,000 individuals, children and families in three main areas:

  • Community Health Services: WIC, Commodity Supplemental Food, Foster Grandparents, Head Start, and Child Development Center.
  • Clinical Health Services: Primary Care, Reproductive Health, Diabetes Management, Immunizations, Migrant Health, Ryan White HIV; Behavioral Health including Counseling Services, Medication Management, Medication Assisted Treatment and Substance Use Counseling and Intensive Outpatient Services and Dental Services.

Job Title: Immunization Coordinator & Primary Care RN

Reports To: Nurse Manager

Areas of Responsibility: This dual role Registered Nurse (RN) position integrates the responsibilities of Immunization Coordinator and Primary Care Nurse. The RN provides comprehensive nursing services in both Immunization and Primary Care Clinics, maintaining full responsibility for the management of CAPWN’s Immunization Program and maintaining program compliance under CAPWN and NEDHHS Immunization Program guidelines. The RN collaborates with nurse and provider teams to support Primary Care Clinic operations, patient education, and health maintenance activities. The RN delivers direct patient care using a provider-led, team-based Patient-Centered Medical Home (PCMH) model, ensuring services that are accessible, continuous, comprehensive, evidence-based, family-centered, coordinated, compassionate, and culturally effective.

Primary Duties:

Immunization Coordination:

  • Oversee, coordinate, and implement all program activities for the Immunization Department.
  • Ensure compliance with Nebraska Vaccines for Children (VFC) and Vaccines for Adults (VFA) policies, procedures, and federal/state guidelines.
  • Develop and manage immunization budgets and workplans; submit required quarterly/annual reports.
  • Maintain accurate vaccine inventory and cold-chain documentation; monitor expiration dates and storage conditions.
  • Serve as the clinic’s Immunization Champion, staying current with CDC immunization schedules, vaccine formulations, and vaccination guidelines for special populations.
  • Provide staff training and community education on vaccine-preventable diseases and immunization best practices.
  • Participate in community outreach and satellite immunization clinics.

Clinical and General Nursing Duties:

  • Perform RN level skills included, but not limited to, patient assessment, medication and immunization administration, medication refills, phlebotomy, specimen collection, wound care, and emergency triage.
  • Provide patient education on medications, at-home care, and health promotion.
Assist with the external referral process and respond to patient inquiries with timely follow-up.
  • Maintain accurate, timely electronic health records and documentation.
  • Participate in quality-improvement, safety, and compliance initiatives.
Assist with coordinating patient appointments and follow up care as needed.
Care-manage chronic diseases in patients by identifying barriers to care and offering information related to specific diseases; facilitate referrals for needed in-house services to improve patient outcomes.
  • Maintain confidentiality of records and communications following CAPWN policy and HIPAA privacy and security policies and procedures.
  • Collaborate effectively with providers, RNs, LPNs, Medical Assistants, and support staff to promote excellent patient outcomes and a positive team environment.
  • Maintain current Basic Life Support (BLS) certification and any required continuing education.

Working Conditions:

  • Standard, well-lit, temperature-controlled clinical environment with standard medical and office equipment available.
  • High volume, fast-paced clinic with frequent patient interaction, average noise level, and occasional irregular hours.
  • Occasional off-site outreach and vaccination events; some travel for training or community clinics or community interaction.
  • Training, meetings and deadline expectations.

Physical Requirements:

  • Ability to frequently bend, stoop, squat, walk and pull drawers.
  • Manual dexterity sufficient to operate office machines and medical equipment.
  • Ability to speak regularly, hear and have close vision to record and communicate appropriate documents and records.
  • Ability to push a wheelchair.
  • Occasionally lifting and carrying up to 30 lbs.

Minimum Qualifications:

Grade: 10

  • Current BLS Certification
  • Valid Nebraska driver’s license and insured vehicle

Grade: 11

  • Meets Grade 4 qualifications plus three (3) years RN experience (preferably in a clinic setting and/or public health nursing).

Grade: 12

  • Meets Grade 4 qualifications plus six (6) years RN or experience (preferably in a clinic setting and/or public health nursing).
