Job summary

Hope Network is seeking an IDDT Case Manager in Saginaw, MI for full-time, hybrid work. The role involves coordinating services to maximize recovery and independence for individuals with dual diagnosis in an outpatient setting, including clinical consultation and home visits.

The position emphasizes community-based care, person-centered planning, and collaboration with multiple providers to support client goals and daily living in the community.

Qualifications

• Bachelor’s degree in a human services field from an accredited institution.
• Experience working with dual diagnosis (substance abuse/mental health).
• Valid Michigan Driver’s License.
• Willingness to work in the community and perform home visits.

Responsibilities

• Work in the community performing home visits.
• Ensure consumer involvement in the Person-Centered Planning Process (PCP).
• Facilitate the implementation of Individual Plan of Service.
• Perform comprehensive, ongoing psychosocial assessments.
• Participate in legal proceedings including petitioning and testifying in clinical emergencies.
• Provide community-based services.
• Facilitate the coordination of consumer intakes, discharges, and transfers.
• Communicate with all service providers in Treatment Plan.
• Facilitate hospitalizations, attend hospital meetings and coordinate with hospital social workers.
• Coordinate and monitor activities such as a job, school, volunteerism, and family caretaking.
• Provide referral to appropriate community resources.
