The position requires QMHP status, with licensed candidates preferred. Strong collaboration, documentation discipline, and ability to coordinate transportation and supports are essential.

Qualifications

• Qualified Mental Health Professional (QMHP) required.
• Bachelor’s Degree in Social Work, Sociology, Counseling, Psychology, Criminal Justice, or other Human Services field (Required).
• Master’s Degree in Social Work or related field preferred.
• Michigan Driver’s License and reliable transportation required.

Responsibilities

• Provides community-based case management services, primary, and behavioral health integrated services.
• Responds to the needs and interests of individuals using assessment practices.
• Provides weekly community-based interventions and facilitates weekly groups.
• Coordinates transportation and medication management for individuals.
• Completes documentation accurately per CNS policy.
