Job summary

Northern Lakes CMH Authority is seeking a Case Manager Specialist to join the ACT/IDDT team in Grayling, MI. You will ensure continuity of services and carry out the six core case management functions in community settings for individuals with serious mental illness and co-occurring conditions.

The role includes crisis intervention, direct clinical care, treatment planning, and collaboration with consumers and staff to achieve recovery goals and community tenure.

Qualifications

• Requires credentialing by NLCMHA in this discipline.
• Master’s Degree preferred in social work, counseling, or psychology; nursing considered.
• Three years post-degree experience in developmental disabilities or mental illness settings preferred.
• Experience as a case manager and with co-occurring populations is desirable.
• State of Michigan professional licensure required or eligible (LSW/LP/LMHP).
• MCBAP certification or eligibility preferred.

Responsibilities

• Complete annual assessment and person-centered planning for high-need clients.
• Facilitate treatment plans addressing housing, relationships, community inclusion, healthcare, and recovery.
• Determine level of care using LOCUS tool.
• Provide crisis intervention and participate in 24-hour on-call coverage.
• Serve as primary SUD therapist for co-occurring clients once credentialed.
• Deliver individual and family counseling and support daily living skills.
• Maintain knowledge of local resources to empower community functioning.
• Participate in daily team meetings and supervision sessions.
• Facilitate transitions of care based on medical necessity.
