Qualifications
Qualifying degree in social work or human service field.
Must be able to drive and possess reliable means of transportation.
Must be able to lift up to 50 lbs. as job may require some lifting.
Responsibilities
Reach out to clients assigned by supervisor and assess their most urgent needs.
Develop a detailed plan of action to meet the client’s needs.
Offer comprehensive psychosocial services to patients.
Consult with external agencies to provide clients with support.
Keep records of clients’ progress throughout the process.
Help clients in meeting basic needs for food and shelter.
Engage and develop trusting relationships with clients.
