Job summary

YAI is seeking a qualified professional to advance intake and transition planning for individuals with IDD. You will collaborate with the Regional Centers, clinical team, and IDT to implement IPP and Person-Centered Plans, and to coordinate services through the transition process.

The role requires a Master’s degree in psychology, social work, or counseling with substantial experience in IDD and forensic contexts, and a track record of linking clients to community supports.

Qualifications

• Master’s degree in psychology, social work, counseling with 5+ years of related experience in IDD behavioral/mental health.
• Extensive experience developing and implementing Individual Program Plans and person-centered plans for people with IDD with criminal/forensic involvement.
• Experience linking IDD clients with complex behavioral/mental health and forensic needs to community supports.

Responsibilities

• Collaborate with DDS and Regional Centers on the Intake and Referral Process.
• Facilitate and lead intake/admission meetings.
• Collaborate with the Clinical Coordinator, Consultants, and IDT to implement IPP and transition plans.
• Assist in risk assessments with the Director and Clinical Director.
• Coordinate with Regional Center Liaison and Service Coordinator to access community services per IPP/Transition Plan.
• Support individuals pre-transition, during transition, and post-transition with monthly visits or more as needed.
• Perform all other duties as assigned.
