Job summary

Independent Living Systems (ILS) seeks a Care Coordinator (IDD) to manage comprehensive, person-centered care for individuals with intellectual and developmental disabilities in Florida. You will assess needs, develop support plans, coordinate services, and monitor progress with multidisciplinary teams, families, and community resources to promote health, independence, and quality of life.

Qualifications

• Requires experience with electronic health records (EHR) and basic computer applications.
• Bachelor’s degree in relevant social services field or equivalent.
• RN or LPN licensure in Florida with experience in IDD settings.

Responsibilities

• Serve as primary contact for the member and authorized representatives.
• Assess member needs and develop person-centered care plans.
• Coordinate services across the continuum and monitor progress to adjust plans as needed.
• Collaborate with healthcare providers, families, and community resources.
• Maintain documentation and ensure compliance with policies and standards.
