Job summary

YAI seeks a qualified Intake and Referral Specialist to collaborate with DDS and Regional Centers on the intake process and to lead admission meetings. You will work with the IPP and person-centered plans, and coordinate transition plans with the IDT.

You will assist in risk assessments, connect individuals with community resources, and provide monthly (or more frequent) support through transition phases. This role emphasizes advocacy and coordinated care.

Qualifications

• Master’s degree in psychology, social work, or counseling with five years' IDD behavioral/mental health experience.
• Extensive experience developing Individual Program Plans and person-centered plans.
• Experience linking IDD clients with complex behavioral/mental health needs to community resources.

Job description

Key/Essential Functions & Responsibilities

• Collaborate with DDS and the Regional Centers on the Intake and Referral Process.
• Facilitate and lead Intake/admission meeting.
• Collaborate with the Clinical Coordinator, Consultants, and other members of the IDT in the development and implementation of the IPP and Person Centered Plan and transition plans
• Assist in conducting Risk Assessments in collaboration with the Director and Clinical Director
• Coordinate with Regional Center Liaison and Service Coordinator on accessing services in the community as per IPP and Transition Plan
• Support the person pre transition, during transition and post transition with at a minimum of monthly visits or more frequently as necessary.
• Performs all other duties, as assigned.

Pay Rate: $87,560.30 annually

Minimum Qualification Requirements including education, experience, licensure/certification, etc. and essential physical capabilities (e.g. lifting, assisting lifting, standing, etc.)

• A Master’s Degree in Psychology, Social Work, Counseling and five years of experience in the past 7 years working with people with IDD with behavioral/mental health concerns with criminal and/or forensic involvement.
• Extensive experience in the development and implementation of Individual Program Plans and person-centered plans for people with IDD with criminal and forensic involvement.
• Extensive experience in linking people with IDD with complex behavioral/mental health and forensic needs to community supports and resources.

Preferred Qualification Requirements (desired requirements beyond MQRs above)

• Credentialed Alcoholism and Substance Abuse Counselor (CASAC) credential
