Job summary

YAI - Seeing beyond disability in Lakewood, CO seeks a Transitional Coordinator to collaborate with DDS and Regional Centers on Intake/referral, facilitate admissions, and coordinate IPPs and transition plans for individuals with IDD and behavioral health needs. The role requires a Master’s in Psychology, Social Work, or Counseling and at least five years of relevant experience, including forensic involvement.

CASAC is preferred.

Qualifications

• Master's degree in Psychology, Social Work, or Counseling and at least five years of experience with IDD and behavioral/mental health concerns.
• Experience in developing and implementing Individual Program Plans and person-centered plans for people with IDD with criminal/forensic involvement.
• Experience linking IDD individuals with complex needs to community supports and resources.

Responsibilities

• Collaborate with DDS and Regional Centers on Intake and Referral processes.
• Facilitate and lead intake/admission meetings.
• Develop and implement IPP and transition plans with the IDT.
• Conduct risk assessments in collaboration with the Director and Clinical Director.
• Coordinate with Regional Center Liaison and Service Coordinator to access community services.
• Support individuals pre-transition, during transition, and post-transition with monthly visits or more as needed.
• Perform other duties as assigned.
