Job summary

YAI Network in Lakewood, CA is seeking a licensed clinical professional with a Master's degree to lead and develop Individual Program Plans and person-centered plans for people with IDD and behavioral/mental health concerns, including those with forensic involvement.

You will manage intake/admission meetings, coordinate with DDS and regional centers, and support transitions with monthly visits or more as needed. Pay is $87,560.30 annually; YAI is an Equal Opportunity Employer.

Qualifications

• Master's degree in a related field and at least five years of experience working with people with IDD with behavioral/mental health concerns, including forensic involvement.
• Extensive experience in developing and implementing Individual Program Plans and person-centered plans for IDD individuals with forensic involvement.
• Experience linking IDD individuals with complex behavioral/mental health needs to community supports.

Responsibilities

• Collaborate with DDS and Regional Centers on the Intake and Referral Process.
• Lead intake/admission meetings.
• Collaborate with the Clinical Coordinator, Consultants, and IDT to develop and implement IPP and transition plans.
• Assist in conducting risk assessments with Director and Clinical Director.
• Coordinate with Regional Center Liaison and Service Coordinator to access community services per IPP and Transition Plan.
• Support individuals pre-, during, and post-transition with monthly visits or more as needed.
• Perform other duties as assigned.
