Job summary

Betty Hardwick Center is seeking a Service Coordinator to support individuals in HHSC Waiver and General Revenue programs. The role focuses on outcome planning, frequent in-person engagement with consumers, families, and providers across five counties, with a strong emphasis on professional assessment and resource coordination.

You will document thoroughly, monitor service delivery, and advocate for consumer rights while working under supervision with the Director of IDD Authority Services.

Qualifications

• Bachelor's or advanced degree from an accredited college or university.
• Associate degree in social, behavioral, human service, or health-related field.
• High school diploma with two years paid/unpaid experience with individuals with IDD.
• Experience with persons with Intellectual and Developmental Disabilities is preferred.
• Must possess a Texas driver's license and qualify for Center insurance coverage.
• Proficient in computer applications (Word, Excel, Outlook).

Responsibilities

• Provide service coordination for individuals in HHSC Waiver and General Revenue programs.
• Outcomes planning and long-term in-person contact with consumers, families, and providers.
• Identify needs, plan with consumer/family to meet outcomes, and monitor service delivery.
• Engage with family, advocates, and providers in a professional manner and document thoroughly.
• Maintain current forms to meet contract requirements.
• Collaborate with service providers and the community while respecting consumer rights.
