Lakes-Regional-Community-Center is seeking a Service Coordinator to assess and verify coordination needs, plan and monitor services, and advocate for consumers. Position requires flexible scheduling, including after-hours, holidays, and weekends, with extensive travel in the local area.

Required education includes a bachelor’s degree in a relevant field or an associate degree with IDD experience; a high school diploma with 2 years of IDD experience is also acceptable.

Qualifications

• Bachelor's degree or advanced degree in any field, or an Associate degree in social/behavioral/health-related field with 1 year IDD experience.
• High School diploma with 2 years paid/unpaid experience with individuals with IDD.

Responsibilities

• Assessment and verification of service coordination needs.
• Service planning, referral, monitoring of service provision.
• Advocacy for consumers.
• After-hours crisis services.
• Documentation of progress.
• Frequent contact with consumers, family members, and service providers.
• Flexible schedule including after-hours, holidays, and weekends.
