Job summary

Central Counties Services is seeking a Service Coordinator to manage consumer plans under Home and Community-based Services and Texas Home Living waivers. The role includes PDP development, meeting facilitation, plan monitoring, and timely documentation in the EHR system.

The coordinator will collaborate with families, providers, and internal teams, and may drive to five counties for in-person contact and planning meetings.

Qualifications

• Bachelor’s degree in Psychology, Social Work or related social services area (required).
• Associates degree in a social, behavioral, health related field or equivalent.
• High School diploma or certificate and two years of related experience acceptable.
• Must have and maintain a valid driver’s license with acceptable driving record.
• Ability to proficiently utilize computer programs, e.g., Word and Excel.

Responsibilities

• Serve as primary Service Coordinator for consumers on caseload in HHSC waivers.
• Develop and evaluate Person Directed Plan (PDP) with consumer and family.
• Facilitate planning meetings and monitor plan progress.
• Complete timely documentation in EHR and maintain notes.
• Drive among five counties to meet with consumers and attend meetings.

Education

Bachelor’s degree in Psychology, Social Work or related social services

Associates degree in social/behavioral/health field

High School diploma or equivalent

Valid driver’s license
