Job summary

Lakes Regional in Greenville, Texas seeks a Service Coordinator to assess needs, plan and coordinate services, and advocate for consumers across a broad range of community-based supports.

The role involves extensive contact with consumers, families, and providers, a flexible schedule including after-hours, and travel within the local area. Bilingual Spanish and English is a plus; a Bachelor's or related degree is preferred.

Qualifications

• Bachelor’s or higher in any field; or an Associate degree in a social/behavioral/health field with IDD experience; or a high school diploma with two years of related experience.

Responsibilities

• Assessment and verification of service coordination needs.
• Service planning, referral, and monitoring of service provision.
• Advocacy for consumers.
• After-hours crisis services.
• Documentation of progress.
