Job summary

Lakes Regional Community Center is seeking a Service Coordinator to assess and verify service coordination needs, plan services, make referrals, and monitor provision. The role requires advocacy for consumers, after-hours crisis support, and thorough progress documentation.

You will communicate with consumers, families, and providers and maintain a flexible schedule, including evenings, holidays, and weekends, to meet client needs.

Qualifications

• Bachelor's degree or advanced degree from an accredited college or university (any field) OR Associate degree in a social, behavioral, human service, or health-related field with at least one year paid or unpaid experience working with individuals with IDD preferred OR High School diploma and two years of paid or unpaid experience with individuals with IDD.

Responsibilities

• Assess and verify service coordination needs.
• Plan services, refer, and monitor service provision.
• Advocate for consumers.
• Provide after-hours crisis services.
• Document progress.
• Maintain frequent contact with consumers, family members, and service providers.
• Work a flexible schedule including after-hours, holidays, and weekends.

Requirements

Bachelor's degree or advanced degree from an accredited college or university (any field) OR Associate degree in a social, behavioral, human service, or health-related field including psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice with at least one year paid or unpaid experience working with individuals with IDD is preferred OR High School diploma or a certificate recognized by a state as the equivalent of a high school diploma, and two years of paid or unpaid experience with individuals with intellectual or developmental disabilities.

Preferred Qualifications

One or more years of work experience serving individuals with intellectual and developmental disabilities; working computer skills. Bilingual in Spanish & English a plus.
