Job summary

PermiaCare is seeking an IDD Service Coordination Team Leader in Midland, TX. This role directs and trains staff delivering IDD coordination and CCCM services, ensuring compliance with regulatory guidelines and performance expectations.

The position involves PASRR Diversion and ECC coordination, supervision of IDD Service Coordinators, and reporting activities to management. Requirements include a Bachelor’s degree in health or human services fields and at least 2 years of relevant experience.

Qualifications

• Education: Bachelor's degree in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistance, gerontology, special education, educational psychology, early childhood education or early childhood intervention (or related coursework).
• Experience: At least 2 years working with individuals with intellectual and developmental disabilities or mental illness preferred.

Responsibilities

• Direct and train staff responsible for IDD Service Coordination, habilitative coordination and CCCM services.
• Ensure monitoring, evaluation and regulatory compliance for service delivery and provider performance.
