Job summary

PermiaCare is seeking an IDD Service Coordination Team Leader to direct and train staff responsible for habilitative coordination and CCCM services. This role ensures regulatory compliance and evaluates provider performance, serving as PASRR Diversion and ECC Coordinator while supervising IDD Service Coordinators.

The position requires a Bachelor's degree in a relevant field and at least 2 years of experience with IDD or mental illness, plus valid driving status and background checks.

Qualifications

Responsibilities

• Supervise IDD Service Coordinators and ensure person-centered service delivery.
• Review staff performance and supervise ongoing improvements in coordination and CCCM.
• Coordinate education and transition planning to explore community living options.
• Provide administrative supervision to ensure HIPAA-compliant documentation and reporting.
• Plan and assign staff workload; participate in hiring and corrective actions as needed.
