Job summary

Emergence Health Network in Texas seeks an experienced professional to provide continuum-of-care coordination for IDD individuals in the community. You will work with LIDDA coordinators, HCS, and external providers to explore community options and coordinate services.

The role emphasizes trauma-informed approaches, collaboration, and timely documentation. A Bachelor's degree in a related field and 1 year of community experience are required; adherence to HHSC performance contracts is expected.

Qualifications

• Requires a Bachelor's Degree in a social/behavioral or human service field and 1 year of community experience.
• Experience with trauma-informed services and DBT is preferred.
• Ability to collaborate with LIDDA staff, crisis teams, and external providers.

Responsibilities

• Provide coverage for evaluator responsibilities during staff vacancies or as needed.
• Coordinate access to community services as a diversion to NF admission.
• Identify community living options and supports to assist individuals living in the community.
• Review admissions within 45–75 days to explore alternatives to NF services.
• Initiate and complete processes for State supported living center.
• Participate in multidisciplinary case consults and drop-in hours.
• Perform outreach to IDD providers and external stakeholders.
• Complete required forms and ensure progress notes within 48 hours.
