A Bachelor’s degree and 5+ years in care management are required, with supervisory experience preferred.

Qualifications

• Bachelor’s degree in a human service, social science, or health field.
• Five years’ experience in Care Management, with at least three years in IDD.
• Proficient in documentation, reporting, and EHR/case management software.

Responsibilities

• Lead and supervise a team of care coordinators to ensure high-quality member-centered service.
• Monitor activities, staff productivity, and caseload performance for timely completion.
• Ensure compliance with policies, contracts, and state/federal regulations through oversight and audits.
• Provide coaching, training, and professional development to strengthen staff skills.
• Review dashboards and data to identify trends and implement improvements.
• Collaborate with providers, families, and interdisciplinary teams to support care plans.
• Promote member satisfaction and continuity of care through responsive service delivery.
