Trillium Health Resources is seeking a Quality Management Specialist II - IDD to ensure HCBS services meet requirements during the plan year and to oversee Innovations Waiver monitoring.

The role collaborates with Care Management to uphold quality support, conducts regular monitoring activities and visits, and travels within the North Central region as needed to ensure adherence to waiver requirements.

Qualifications

• Must meet Qualified Professional (QP) status per NC requirements through listed paths.
• RN licensed in NC with MH/DD/SA experience is acceptable.
• Master's or Bachelor's in human services with MH/DD/SA experience meets criteria.
• Four-year degree in non-human services with MH/DD/SA experience can qualify.

Responsabilités

• Monitor HCBS services for compliance with state standards and Medicaid regulations.
• Promote service satisfaction through ongoing communication and timely follow-up.
• Perform announced/unannounced monitoring visits to members, including nights/weekends as needed.
• Ensure monitoring is performed across all settings (in-home, community, etc.).
