Job summary

Community Services Group is seeking a Program Specialist to coordinate assessments and develop Individual Support Plans (ISPs) for adults with IDD in our community home services. The role focuses on promoting independence across daily living, communication, and adaptive skills, in collaboration with a team of professionals.

Responsibilities include training staff, leading team meetings, and ensuring plan progress is documented, with opportunities for professional growth and a two-week

Qualifications

• Master’s degree or higher with 1+ year IDD experience or
• Bachelor’s degree with 2+ years IDD experience; or
• Associate degree with 4 years IDD experience.

Responsibilities

• Train employees including new-hire onboarding and orientation.
• Coordinate initial and ongoing assessments of individuals.
• Develop, review and implement Individual Support Plans (ISPs).
• Collaborate with behavior specialists and therapists for plan progress.
• Lead multidisciplinary team meetings and document outcomes.
• Create educational materials and support staff with Person-Centered Planning.
• Report incidents per policy and manage incident documentation.
