Job summary

Community Services Group is seeking a Program Specialist to coordinate assessments and develop, review, and revise the Individual Support Plan (ISP) to foster independence across daily living, communication, and community skills. You will implement goal plans in collaboration with a team and report to the Program Director in our Adult IDD Community Home Services.

Qualifications

• Master’s or higher with IDD experience required as listed.
• Alternate tracks with bachelor or associate degrees and IDD experience accepted.

Responsibilities

• Coordinates and completes initial and ongoing assessments.
• Develops, reviews, and ensures ISP implementation.
• Leads multidisciplinary team meetings for the service plan.
• Develops educational materials and supports staff training.
• Reports incidents per policy and maintains documentation.
