Job summary

Community Services Group in Lancaster, PA is seeking a Program Specialist for IDD Community Home Services. The role focuses on completing and coordinating assessments, developing and reviewing Individual Support Plans (ISPs), and implementing goal plans to foster independence in areas such as daily living, communication, and adaptive behavior.

The position is full-time (40 hours/week) with two-week paid orientation conducted at the Mountville office.

Qualifications

• Master’s degree or above in a field related to IDD with 1 year of relevant work with persons with IDD
• Bachelor’s degree with 2 years of relevant work with IDD
• Associate’s degree with 4 years of relevant work with IDD

Responsibilities

• Provide training to employees including on-the-job training and orientation.
• Coordinate and complete initial and ongoing assessments.
• Coordinate and develop, review, and ensure implementation of individual support plans (ISPs).
• Work with professionals involved in services to ensure plans are implemented and progress documented.
• Lead multidisciplinary team meetings relating to the individual’s service plan.
• Develop and present educational materials and assist staff with Person-Centered Planning and ISPs.
