Job summary

Active Day is seeking an ODP Program Specialist to support adults with intellectual disabilities by assessing needs and developing goals aligned with each ISP. You will collaborate with ISP meetings, monitor progress, and ensure services comply with state and company policies.

In this role you will maintain documentation, coordinate with Center Director and staff, and help deliver person-centered care while upholding safety standards and regulatory requirements across center operations.

Qualifications

  • A program specialist shall have one of the following groups of qualifications.
  • An associate's degree or 60 credit hours from an accredited college or university and 4 years of work experience with persons with disabilities.
  • A bachelor's degree from an accredited college or university and 2 years of work experience with persons with disabilities.
  • A master's degree or above from an accredited college or university and 1 year of work experience with persons with disabilities.
  • Minimum one (1) year experience working with adults with intellectual disabilities preferred
  • Must be detail-oriented with exceptional organizational skills to maintain compliance.
  • Strong communication and relationship building skills.
  • Ability to multi-task and manage competing priorities.
  • Working knowledge of Microsoft Office (Outlook, Word and Excel).

Responsibilities

  • Coordinate and complete assessments and develop member goals
  • Assess member progress with goals and objectives set forth in assessments
  • Participate in the development of member ISPs, including updates/revisions
  • Act as member ISP lead relating to development, annual update and revision of ISP
  • Attend ISP meetings
  • Report discrepancies in the ISP and changes related to member needs
  • Ensure services provided are in accordance with ISP
  • Document monthly participation and progress and suggest revisions as needed
  • Develop and implement provider services with Center Director
  • Coordinate training of teammates on health and safety needs
  • Perform required documentation to ensure regulatory compliance
  • Support Center staff and members daily and communicate with support teams
  • Maintain filing system and prepare case notes and reports upon request
  • Attend meetings to address service concerns and new member enrollment
  • Support members with personal care services and mealtime support
  • Intervene in crises; maintain proficiency in client techniques and represent Active Day’s culture
  • Maintain licenses and certificates; demonstrate knowledge of progressions in the field
