Responsibilities

• Maintain and update individual Day plans and ensure accuracy.
• Attend ISP meetings and coordinate with the IDT.
• Develop goals for individuals and track progress monthly.
• Prepare monthly progress notes for participants.
• Ensure compliance with state and agency requirements in all records.
• Supervise direct support professionals and coordinate with families.
