Job summary

AHRC New York City is seeking a Full-Time Treatment Coordinator/QIDP for its Queens ICF residence in Jamaica. The role coordinates the training and habilitation program of residents with residential staff, educational and vocational services to ensure active, meaningful daily engagement.

The ideal candidate has a Bachelor's degree in human services, strong leadership and organizational skills, and the ability to coordinate a multidisciplinary team.

Qualifications

• Bachelor's Degree in the human services-related field
• Must have a basic knowledge of all clinical areas that comprise the interdisciplinary team for individuals with intellectual disabilities
• Must have a good working knowledge of direct care responsibilities
• Must possess leadership abilities and be able to coordinate a team of clinical and management staff
• Must have excellent organizational skills and good writing ability
• Must possess the ability to deal tactfully, professionally, and effectively with residents, personnel, family members, visitors, government agencies, and the public

Responsibilities

• Coordinate the training and habilitation program of residents with staff and related services
• Generate Needs/Capabilities list annually with updates
• Ensure each resident has an activity schedule and ongoing active treatment
• Advocate for residents in areas of cleanliness, clothing, bedrooms, and toiletries
• Complete reviews of each resident's treatment plan with progress summaries
• Liaise with day/workshop programs to coordinate services between day and residential components
• Ensure IPPs and day notes are on file
• Train staff in behavioral interventions and active treatment areas
• Coordinate Annual Review process and prepare annual reports
• Generate goals, objectives, and interventions for indicated needs
