Job summary

Eihab Human Services in Springfield Gardens, NY is seeking an experienced Client Coordinator to oversee Day habilitation services for individuals with developmental disabilities. You will lead interdisciplinary teams, supervise staff, ensure compliant intake, and drive delivery of individualized plans in a supportive, therapeutic environment.

Responsibilities include coordinating Life Plans, monitoring service provision, and ensuring documentation meets state and agency standards.

Qualifications

• Customer interaction with individuals professionally and ethically; understand and implement service plans; complete required documentation clearly and timely.
• Previous experience with IDD/Mental Illness and proven team management experience.
• Competency in Microsoft Word and basic computer skills.
• Experience in Day Support Programs for IDD or Special Education environments is preferred.
• Passion for working with people with intellectual disabilities.

Responsibilities

• Maintain all individual Day plans and update as needed.
• Attend all Individualized Service Plan meetings and reviews.
• Develop goals for individuals, monitor progress, and document outcomes monthly.
• Assist individuals and advocates in obtaining services outlined in Day Habilitation Plan.
• Complete monthly progress notes for all participants.
• Maintain Day habilitation plans and update as needed.
• Identify trainings for Direct Support Professionals to ensure adequate services.
• Communicate with parents, family, and residential staff about issues and updates.
• Conduct regular program observations to ensure goal implementation.
• Ensure Life Plans, IPOP, Assessments and related records comply with OPWDD and OMIG requirements.
