Job summary

RHA Health Services, LLC in Trenton, TN is hiring a Case Manager / Program Manager / IDD Qualified Professional to coordinate person-centered plans and oversee interdisciplinary services. You will interview individuals and guide goals across health, psychological and independence outcomes.

You will supervise plans, monitor progress, and collaborate with nurses, direct care staff, and managers to ensure safe, effective community living and quality care.

Qualifications

• Bachelor’s degree in Social Work, Social Services, Human Services or Special Education required.
• 2+ years experience with intellectual/developmental disabilities or related mental health support.
• Knowledge of relevant regulations governing services.

Responsibilities

• Assist individuals to identify service needs via direct interviews.
• Coordinate and monitor person-centered plans and outcomes.
• Supervise and write plans based on assessments and observations.
• Oversee residential activity and community integration to ensure safety.
• Recruit, hire, train, and supervise Group Home Managers.
