Job summary

RHA Health Services, LLC in the United States is hiring for a Case Manager / Program Manager / IDD Qualified Professional to guide the development of person-centered plans and programs through interviews with the people we serve.

The role centers on coordinating services, supervising plans, and monitoring progress toward personal and clinical outcomes, with emphasis on collaboration with nurses, Direct Care Associates, and facility leadership.

Qualifications

• Bachelor’s degree in Social Work, Social Services, Human Services or Special Education required; Master’s degree a plus.
• Minimum 2 years experience working with persons with intellectual and/or developmental disabilities; 1-year if paired with Master’s degree.
• Valid driver’s license, auto insurance, and reliable transportation.
• Experience writing person-centered plans and supervising others is preferred.

Responsibilities

• Assisting each person we support to identify and communicate his or her requests and needs for services and supports through direct interviews.
• Supervising, integrating and coordinating person-centered plans and monitoring progress towards personal, clinical and functional outcomes.
• Initiating periodic reviews, investigations, modifications, and adjustments by soliciting feedback from the people we support as needs/goals change.
• Reviewing assessments completed by the people we support and other team members to help meet their full potential.
• Supervising and writing the person-centered plan based on assessments, interviews, and observations.
• Overseeing residential activity to ensure people we support are healthy and safe.
• Visiting vocational centers, residential homes and other service sites to assess plan implementation.
• Reporting to the Administrator and collaborating with colleagues to maintain communication and improve procedures.
• Monitoring activities to ensure quality assurance and state survey improvements.
• Helping recruit, hire and train Group Home Managers.
• Serving as the link between the people we serve, families and guardians to gain approval for services.
