Job summary

RHA Health Services, LLC is seeking an organized and positive IDD Case Manager/Program Manager II to coordinate and monitor services for individuals with intellectual and developmental disabilities and mental health needs. You will supervise staff, train Direct Care Associates, and ensure person-centered plans are implemented across residential and day settings.

The ideal candidate brings strong communication, interviewing and writing skills, and a commitment to quality outcomes.

Qualifications

• Bachelor’s degree in social work or related field required.
• Master’s degree is a plus and preferred.

Responsibilities

• Coordinate and monitor services to meet personal outcomes.
• Supervise staff and write person-centered plans.
• Assist individuals in identifying needs and requests for services.
• Maintain documentation and reporting for quality assurance.
