Job summary

Socket.dev is seeking a Target Case Manager to assist families in planning and coordinating I/DD services in Wichita, KS. You will attend IEP meetings as an advocate, provide community resources, and create a Person-Centered Support Plan tailored to each child’s needs.

The role requires a Bachelor's Degree in Social Work or a related field and at least 6 months of full-time experience in human services, with experience serving children with special needs preferred.

Qualifications

• Bachelor’s degree required in Social Work or related field.
• Minimum of 6 months’ full-time experience in Human Services.
• Prior experience with children with special needs preferred.

Responsibilities

• Provides information on IDD Services.
• Assists with service planning and coordination of IDD services.
• Assist clients with needs assessment on an on-going basis.
• Organize, prioritize, and perform work tasks within deadlines and policies.
• Deal effectively with clients, community professionals, and co-workers in a professional manner.
• Comply with rules of Conduct for Case Managers Serving People with Developmental Disabilities.
• Performs other tasks as assigned.
