Job summary

Community Support Services of Missouri (CSS) in Joplin, MO is seeking a Case Manager to support individuals with developmental disabilities. The role focuses on planning, advocating for services, and coordinating with a multidisciplinary team to determine the best supports.

You will develop Person-Centered Service Plans (PCSP), monitor progress per DMH directives, and assist in training and quality assurance activities.

Qualifications

• Bachelor’s degree or RN license required.
• Strong knowledge of case management methods, principles and techniques.
• Ability to manage a caseload of clients with developmental disabilities and maintain records.

Responsibilities

• Interviews clients, families, and others; schedules assessments and gathers information.
• Develops Person-Centered Service Plans (PCSP) with interdisciplinary teams.
• Monitors PCSP per DMH Directive and maintains logs and timelines.
• Attends staff meetings, trainings, and QA activities.
• Represents the agency at DMH workgroups and completes required documentation.
