Job summary

Rainbows-United, Inc. is seeking a Target Case Manager to help families plan and coordinate I/DD services. This role attends IEP meetings as an advocate and develops a Person-Centered Support Plan for the child’s needs.

You will connect families with community resources, work with professionals, and ensure compliance with standards. The position is full-time in a professional office setting, with some travel across the state.

Qualifications

• Bachelor’s degree in Social Work or related field required.
• Minimum of 6 months full-time experience in human services.
• Prior experience with children with special needs preferred.

Responsibilities

• Provides information on IDD services.
• Assists with service planning and coordination of IDD services.
• Assist clients with needs assessment on an ongoing basis.
• Organize, prioritize, and perform work tasks within deadlines and constraints of policies, procedures, and processes of regulatory entities.
• Deal effectively with clients, community professionals, and co-workers in a professional manner.
• Comply with rules of Conduct for Case Managers Serving People with Developmental Disabilities.
• Performs other tasks as assigned.
