Job summary

EP!C is seeking a qualified QIDP to provide person-centered clinical case management for individuals with intellectual and developmental disabilities. The role includes developing, implementing, and monitoring individualized service plans, ensuring federal and state regulatory compliance, and leading staff in adherence to policies and procedures.

Qualifications

  • Bachelor's Degree preferred in Human Services or education and two years’ supervisory experience required.
  • Supervisory experience is essential.
  • One year experience with persons with developmental disabilities.
  • Ability to read and comprehend simple instructions, short correspondence, and memos.
  • Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization.

Responsibilities

  • Support the agency's mission, vision, values, and strategic plan philosophy.
  • Manage all responsibilities based on the agency's mission, using individual choice, and all appropriate policies, procedures, and regulations.
  • Conduct oneself in a way that reflects the highest standards of professional courtesy and ethical practices to people served, staff, guardians, families, and all outside contacts.
  • As the 24/7 QIDP coordinate Community Day Services based on individual needs and preferences. Monitor service delivery and Individual satisfaction with programming and offerings.
  • Coordinate health services based on individual needs and licensure requirements. Schedules for psychiatric appointments, discharge staffing's, and community appointments are kept, and appropriate documentation is forwarded to the appropriate nursing staff.
