Job summary

The Association For Individual Development (AID) seeks a Case Manager QIDP to oversee a caseload of up to 20 clients in our I/DD service programs, coordinating internal and external services to support progress and community inclusion. You will maintain client case records in the electronic system (Cx360), lead service planning meetings, and ensure DHS, HCBS, and CARF compliance.

Direct service delivery, advocacy for client rights, and documentation accuracy are essential.

Qualifications

• Bachelor's Degree in Human Services required.
• At least 1 year of proven experience working directly with individuals with developmental disabilities; QIDP preferred.
• Ability to coordinate services and maintain documentation per DHS/HCBS/CARF standards.

Responsibilities

• Maintain assigned caseload of up to 20 clients and manage client case records in the electronic system (Cx360).
• Schedule and lead service planning meetings annually and as needed per licensure timelines.
• Coordinate Health, Behavioral Intervention, Community Day, Behavioral Health, and Employment Services based on client needs and licensure requirements.
• Ensure consent, rights, and privacy are maintained; monitor service delivery and client satisfaction; provide direct service as needed.
• Review records, monitor adherence to budgets, and ensure timely documentation and reporting to supervision.
