Qualifications

• Minimum two (2) years of experience with individuals with intellectual developmental disabilities.
• Bachelor's degree in social work, sociology, psychology, gerontology, or related field.
• RN or LPN license with relevant experience preferred.

Responsibilities

• Serve as primary contact for members and authorized representatives.
• Assess member needs and develop person-centered support plans.
• Coordinate services across the continuum and liaise with providers.
• Monitor progress and adjust care plans as needed.
• Maintain documentation and ensure compliance with policies and quality standards.
